Copies of death certificates are on file in the Town Clerk's office where an individual died, the Town Clerk's office where an individual resided at the time of death, and the State of Connecticut Department of Vital Statistics in Hartford. Expedited processing service is also available for an additional fee for requests ordered on-line via VitalChek.  VitalChek, a third party vendor approved by the CT DPH, allows customers to order certificates from Farmington through either a toll free telephone number or on-line.

VitalChek can be reached either through its website, www.vitalchek.com, or by phone at 1-866-809-0839.  An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.  Only requests ordered via Vitalchek allow customers to pay for certificates with a credit card and to track the status of ordered certificates that are sent by UPS overnight mail.
